Materials Survey

Per NIH Guidelines, projects that include the use of any known biohazardous risk, any Recombinant Nucleic Acid materials, and/or materials that may pose a risk must be reviewed by the Institutional Biosafety Committee (IBC). This survey will help determine whether or not this project will require review. 

Please check all of the following materials that will be used in this research project.

1. Microorganisms/potentially infectious material (e.g., viruses, bacteria, yeast, fungi, parasites, prions)(Required)
2. Whole animals/animal material (e.g., introduction of biologicals/chemicals into animals, use of animal cell lines and/or tissues)(Required)
3. Arthropods (e.g., insects, spiders, crabs, lobsters, shrimp)(Required)
4. Recombinant and/or Synthetic Nucleic Acid molecules (e.g., bacterial, plasmids, viral vectors, chemically synthesized NA molecules)(Required)
5. Biological toxins (e.g., cholera toxin, pertussis toxin, diphtheria toxin, tetrodotoxin)(Required)
6. Human or non-human primate material (e.g., blood, fluids, tissues, primary/established cell lines)(Required)
7. Human gene transfer/therapy (e.g., DNA vaccines)(Required)
8. Nanoparticles or other materials that form nanoparticle sized materials (e.g., use of Jet Pei or Poly L Lysine)(Required)
9. Plants (e.g., toxic/transgenic plants)(Required)
